Welcome to transcendent Case Management & consulting, LLC. 
We  are a forward thinking, Case Management and Social Service  Consulting agency, providing innovative services to assist individuals and social service agencies in reaching their goals. A family owned agency, founded in 2016,  we provide parallel services, such as, support, advocacy, and linkage on an individual level with assessment & planning, coaching,  and leadership development on an organizational level in order to generate more positive experiences while utilizing community services. 


We believe in a multifaceted approach to human services, taking a faith-based approach in working both on an individual level and organizational level in order to assist people in rising above barriers and reaching goals and milestones that they never imagined was possible. We do this by  connecting treatment of individual clients and families with providing consulting to social service agencies in effort to enhance service delivery and service utilization. Transcendent is  unique in that it is the only social service agency in the city that offers such an approach.


Simply, Transcendent helps individuals thrive in their daily environment and equip agencies with the strength of caring.

As people with mental illness, substance addiction, and at-risk behaviors face varied and complex issues, a greater demand are placed on social service agencies to provide a better model of care, even in a climate economic insecurity. ​At  Transcendent Case Management & Consulting, LLC, we do not believe that circumstances facing the mentally ill and others at-risk and vulnerable occur in a vacuum. Instead, we believe that their circumstances have the following root causes:
  • Negative social, political, economic, and environmental events
  • Lack of resources and knowledge
  • Biases, both internal (the client) and external (the community), including hopelessness, stigmatization, and shame
  • Fear of change

​For members of this population, assistance in addressing and countering these causes is crucial for them to lead a better quality of life. The most effective and efficient way of accomplishing this is to optimize our support to community social service agencies and agents, via consulting and business analysis, as they strive to provide the needed aid and services to the most vulnerable in society. This includes training & mentoring for staff members, team building, leadership development, and cultural awareness. ​This is the essence of being in a working partnership.

In addition to consulting with social service agencies and agents, we maintain a viable relationship with the client's family and friends, advocates, healthcare professionals, personal budget brokers, and others, in order to deliver quality, coordinated care.

We accept Medicaid and Private Pay. We also accept Self-Pay - sliding scale fees available for those who qualify.
