How Can Case Management/Community Psychiatric Support (CPS) Empower Me?
The intent of Case Management/Community Psychiatric Support (CPS) services is to help clients in becoming self-supporting while assisting them in building a strong network of providers and accessing resources according to their unique and individual needs. The effect of your practitioners interventions and collaboration with client and other key agencies will reduce stressors in a client's life and eliminate barriers to recovery.
Do I Need a Case Manager (and Peer Support Technician) If I Am Seeing a Therapist?
Typically, a Therapist's or Counselor's role is to provide evidence-based clinical treatment, advice and tools for coping with everyday situations, relationships, health challenges or disabilities that interfere with a client's quality of life. A Case Manager/CPS expands upon the Therapist's/Counselor's role by helping clients reaches his or her optimum level of wellness and functionality within his or her daily environment. Case Manager/CPS will provide assessments, advocacy, serve as the liaison between you, the client, and agencies/other professionals. Case Managers/CPS can assist you in accessing food, housing, transportation services, arranging appointments, medication management, etc.
In conjunction with our Case Management/CPS services, our Peer Support Technicians work specifically with adolescents and young adults, age 10 - 24 years old, too promote continued therapeutic growth through role-modeling, teachable moments, and community engagement. At a minimum, clients of Transcendent Case Management & Consulting, LLC are required to receive Case Management and/or Community Psychiatric Support Services.

What Will My Case Manager Do?
Case Manager/CPS provides community-based, client-centered case management services. Some of the major duties of a Case Manager/CPS are:
- Develop, Person-Centered strengths based treatment plans initially and annually reflecting consumer's life goals and individualized choices.
- Develops a discharge plan for each client within their caseload for continuum of care and assists clients in coordination of after-care planning.
- Advocate with approved third parties with a release of information on behalf of client. Including letters to referral agencies, courts/probation.
- Creates and facilitates Didactic, Psychoeducational sessions to assist in treatement and teach valuable life skills.
- Provide behavioral and emotional support to clients to facilitate living with maximum level of independence in their daily environment. Provide training to client's family/caregivers.
- Promotes and assists patients in developing self-help skills.
- Provide stage- based interventions to consumers experiencing co-occurring conditions, and utilize motivational interviewing skills. Make referrals as needed to substance abuse counselor or substance abuse treatment provider and maintain communication in regards to treatment progression.
- Provide active outreach to persons who may be reluctant to engage and begin to develop a working alliance towards understanding their individual concerns and needs
Can I Opt to Only Receive Peer Support Services?
While you may elect to only receive Case Management/CPST services, our Peer Support Technician services can only be elected if you are receiving Case Management services. Once Case Management services concludes, Peer Support services will also conclude.

Yes, we accept Medicaid! We accept Medicaid fee-for-service and CareSource. We are working with other Medicaid Managed Care Organizations in order to expand our payment options for our clients.
We do not accept private insurance. We do accept private pay (via cash, PayPal or *credit cards. Sorry, no personal checks. We offer reduced-fees for clients meeting certain financial criteria.
